We are seeking team members with strong time management and organizational skills, who are motivated, and excel at working with others. As a Care Manager you will be providing Supports Coordination for individuals with Intellectual Disabilities and/or Autism. You will assist individuals and their families with identifying needs, developing a plan of support, and then coordinating and monitoring supports and services. We are guided by principles of Everyday Lives to support individuals in making choices, achieving outcomes, and being actively involved in their community.

JOB DUTIES AND RESPONSIBILITIES:

  • Work with individuals and their families to identify needs and develop desired outcomes.

  • Locate and coordinate services to support the individual’s needs and goals.

  • Work with individuals, their family, and service providers to develop an Individual Support Plan that is person-centered and reflective of the individual’s desired outcomes.

  • Monitor the individual’s health, safety, progress, and needs.

PHYSICAL AND SENSORY REQUIREMENTS:

Must have ability to sit for 2 total hours per day, and up to 1 total consecutive hour. Must be able to stand for 8 total hours per day and up to 2 total consecutive hours. Must be able to walk 6 total hours per day and up to 10 total consecutive minutes. Must have the ability for hearing as it relates to normal conversation, high frequency and low frequency hearing sounds. Must have the ability for seeing as it relates to general vision, near vision, color vision and peripheral vision.

EDUCATION:

  • Bachelor’s Degree which includes, or is supplemented by at least 12 college credits in Sociology, Social Welfare, Psychology, Gerontology, Criminal Justice, or other related Social Science.

TRAINING AND EXPERIENCE:

  • Computer Proficiency

  • Excellent Time Management and Organizational Skills

  • Positive and Friendly Demeanor

  • Ability to perform physical requirement of the job description, with or without accommodation.

WORK SCHEDULE:

  • Full-Time

  • 8 Hour Shift - Flexible

  • Monday to Friday

  • Opportunity for flexibility and hybrid work schedule after successful completion of training

OTHER:

  • Must possess and maintain a valid Driver’s License along with a driving record in compliance with St Luke’s Penn Foundation Policy.

  • Must be able to travel off-site for various meetings and monitoring.
