R.A.I.N. Total Care Inc. Home Health Aide (F/T & P/T) in Bronx, New York

Position Summary: A home health aide is a person who carries out health care tasks under the supervision of a registered nurse or licensed therapist. Direct report is to the Coordinator for scheduling and case assignment. S/he may also provide assistance with personal hygiene, housekeeping and other related supportive tasks. The HHA provides such care to a patient in his/her place of residence.

ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. ESSENTIAL CLINICAL FUNCTIONS: Performs ONLY those functions listed on each patient's care plan: Personal Care - assists with: Tub, bed, sponge bath or shower Oral hygiene - care of teeth and mouth Care of hair Shaving Care of nails, except for cutting of nails Routine skin care/lotion, massage Positioning and turning of patients who cannot assist self Elimination Dressing Eating Weigh patient 2. TREATMENTS, AFTER DEMONSTRATING COMPETENCY TO AN RN, ASSIST WITH: Ambulation, including guarding and holding patients who have difficulty Transfer of patients who cannot assist self: From bed to chair/wheelchair With bedpan or to bathroom To sitting position With Hoyer lift Exercise program (excluding passive exercises) Non-sterile dressing change Prosthesis Special equipment Vital signs: temperature, pulse, respiration, and blood pressure Intake and output Specimen collection: urine, stool and sputum Cooling sponge bath Condom (Texas) catheter Care of indwelling and straight catheters Remind to take/assist with medications Test Urine Simple soaks Special skin care Use of rehabilitative devices Application of binders, ace bandages and other supports 3. Homemaking and Nutritional Support - assists with: Meal planning and preparation Serving and feeding patient Linen change Laundry Light housekeeping Grocery shopping Dishwashing Shopping Payment of bills Essential errands Handle mail

The following functions are performed only after demonstration of competency to an RN on a case-by-case basis AND when the patient is self-directing. Care of catheter or colostomy drainage bag Change of colostomy appliance Other allowable activities listed in the New York State Department of Health o Matrix as determined on a case-by-case basis by the coordinating nurse in consultation with the administrator Follows universal precautions whenever giving any aspect of patient care Responds to the physical, emotional and development needs of patient Follows emergency procedures in the event of any incident, i.e., accident, injury or significant change in patient's condition.

ESSENTIAL ADMINISTRATIVE FUNCTIONS: Respects patient rights always Maintains confidentiality Records each activity performed on each case daily Reports any incident or change in the condition of patient or environment immediately Submits duty sheets and recordings on a timely basis Follows all appropriate agency policies Attends at least 12 hours of in-service education annually Provides all information required for the maintenance of her/his personnel record. Shows interest, asks questions and seeks help as indicated; is receptive to supervision Develops relationships and are cooperative with patient, family and supervisor Is productive and uses time efficiently Is self-reliant and plans appropriately Maintains appropriate professional appearance Follows instructions, is punctual and attendance is acceptable Cooperates with supervisor and with patient/family

MINIMUM QUALIFICATIONS: Demonstrated physical ability to perform lifting, carrying, pushing and pulling activities at a medium strength level Valid Home Health Aide Certificate Clearance through the State Criminal History Record Check Experience in personnel or casework Demonstrate 6th grade literacy level Good communication skills, Bilingual/ Spanish, Russian and other languages as needed in our diverse community preferred
