This is a grant-funded position budgeted for 9-12 months of service. Project Overview: For this project, incumbent will be responsible for work related to two grants. There will be a focus on Breast Health, encouraging community members to receive screening mammography and promoting breast health. In addition, incumbent will focus on identifying health-related social needs and work to meet those needs through available resources and community connections. The incumbent will serve as part of the Mission Services/Community Health team and will be involved in many community health related programs. The community health worker will: * Connect patients/community members and families to resources and services that address their social influencers of health. * Provide referrals and connection to service. * Participate in bi-weekly food distribution. * Attend health fairs and other community-based health promotion events. * Participate in Medical Mission program. * Assist with CHW caseload as needed to ensure continuity of care. * Ongoing data entry tracking client progress and interactions.
