Job Title:      SUPPORTS COORDINATOR -- FAMILY SERVICES/CHILDREN

Job Summary: The Supports Coordinator assigned to the Family Services team coordinates supports and services for children/adults living with their legal/biological/adoptive-foster families. The Supports Coordinator serves as the primary advocate and broker of services within the system for the people he/she/they support. He/she/they work within the mental health system and the community to ensure the people supported have maximum access to and use of the resources available. The Supports Coordinator focuses efforts on connecting the children/adults supported to networks of family, friends, and meaningful relationships, and develops strategies to coordinate the individual's personal resources with entitlements and special programs.

The Family Service Supports Coordinator provides these services while adhering to the principles Person Centered Planning, Family Centered Planning, the Michigan Mental Health Code, Detroit Wayne Integrated Health Network (DWIHN) and Michigan Department of Health and Human Services (MDHHS) requirements, Self-Directed Services Technical Advisory, CLS Guiding Principles, Code of Ethics, and professional code of ethics.

Duties and Responsibilities:

The Supports Coordinator assigned to the Family Services team is responsible for the following:

Meets with the individual in accordance with the amount, scope, and frequency noted in their Individual Plan of Services (IPOS) and additionally, as the individual's needs and choices dictate.

Assists the families, along with the children and adults, in building circles of advocates, friends, family involvement, and identification of life choices as well as strategies to fully achieve their goals.

Works collaboratively and cooperatively with CLS employees both within and outside of the Family Services Department, the overall Supports Coordination Department as well as outside entities to ensure services are met for individuals supported.

Completes all documentation with a high degree of quality and in a timely manner consistent with CLS agency requirements and other external funding sources.

Assists the individual/family in accessing entitlements such as Social Security, Family Support Subsidy, Children's Specialized HealthCare Services, Adoption Subsidy, Children's Waiver (for those Supports Coordinators who work with Children on the Waiver), Autism Benefit Waiver, Wraparound Services and Home Help.

Links, coordinates, and provides monitoring of services as identified in the IPOS and as communicated by the person, their family and circle members.

Assists with pro-active planning on behalf of the individual related to potential crises situations and provides crisis resolution support as needed.

Assists the individual and family to coordinate and secure services needed to address any health concerns. Coordinates health and safety strategies, including planning related to emergency needs and coordination with Medicaid Health Plans and other health care providers. Coordinates discharge planning as needed for hospitalizations.

Assists the individual to identify goals and develop strategies to support the goals both logistically and financially. Assists with goal development, fulfillment, plan implementation, and follow-up per funding requirements.

Assists the individual to become an active and contributing member of their community.

Assists the individual with the development of self-advocacy skills.
